APPLICATION CHECKLIST AND GUIDELINES
An applicant to the Creighton University Pipeline to Success Health Careers Opportunity Program must:
Be a U.S. citizen, non-citizen national, permanent resident, or foreign national who possesses a visa permitting permanent residence in the United States.
Currently enrolled in high school, community college, a 4-year college/university or health professional school.
Must be economically OR educationally disadvantaged.
Must be interested in a health career or the health sciences.
Must meet additional HCOP eligibility criteria.
Must possess a 2.5 GPA or greater (cumulative) on a 1.0 – 4.0 scale.
The application form is completed online. When you click "submit," your application will be automatically sent to firstname.lastname@example.org An application will not be evaluated until it is complete and includes the following:
Photographs: Photographs are used for identification purposes only. Please include a recent digital photo in the space provided on the application form.
Technical Standards (Question 16): Please review the

Personal Statement (Question 19): The personal statement should provide the admissions committee with:
Post-Baccalaureate Program in Medicine application
Verified American Medical College Application Service (AMCAS) application
Three letters of recommendation
Official transcripts for all post-secondary schools -
transcripts sent to AMCAS do not fulfill this requirement
Applicants are asked to submit a copy of their current and verified AMCAS Application report to medical school.
Please submit scores for all attempts at the MCAT not reported in the AMCAS application above. These scores may be copies of reports received by the applicant.
Letters of Recommendation
Three letters of reference from teachers and/or community leaders are required for admission. Please request that letters be sent directly to the Office of Medical Admissions and that they address the following:
Information about your future goals as they relate to the Post-Baccalaureate Program in Medicine and your career in medicine
Information on how your disadvantaged status may have kept you from meeting your educational goals
Please have letters of recommendation mailed to the same address as your transcripts (below), or emailed to: email@example.com
We are aware that AMCAS has already received your transcripts; however, Creighton University also needs to receive official copies. Therefore, all applicants are to submit official transcripts of all academic work. If you have earned a degree from an institution, the transcript must reflect that status. If you are currently enrolled in a degree program we ask that you submit a transcript and then request that a transcript be sent after the posting of your grades/degree.

You will be notified by email when your file is complete. If you do not receive an email notification within 10 days of the date you feel that your application should have been complete, please contact HS-MACA at: firstname.lastname@example.org to ascertain what items have not been received. HS-MACA will attempt to notify you by email about missing items in your file. However, you are responsible for ensuring that your file is complete. Your file cannot receive consideration by the Committee on Admissions until it is complete. The deadline for the receipt of all application materials in the HS-MACA Office is at the close of business (4:30 p.m. CT) on
January 15, 2019.
STIPENDS AND SCHOLARSHIPS
Q: If my AMCAS application is online with Creighton do I still need to send all required documents?
A: Yes, you are required to submit the necessary documentation. For further information, please contact the Health Sciences Multicultural and Community Affairs Department at 1.855.770.2700.
Q: How can I apply to this program?
A: Online applications are available on this site from September 1 until January 15.
Q: What is an LCME-accredited school?
A: The Liaison Committee on Medical Education (LCME) is the nationally recognized accrediting authority for medical education programs leading to the MD degree in the United States and Canada. The LCME is sponsored by the Association of American Medical Colleges and the American Medical Association.
Q: If I apply/get accepted to the post-baccalaureate program will I have to withdraw my medical school application?
A: Yes, all medical school applications must be withdrawn by the start of the program.
Q: Is your program a career change?
A: No, an academic enhancer program. The goal is to prepare students for admission to medical school.
Q: Can I postpone my acceptance until next year due to circumstances beyond my control?
A: Unfortunately, there is no deferment for the program. If you are accepted for admission and choose not to enroll, you will need to reapply next year.
Q: I would like to know whether my status meets Creighton’s criteria in determining a disadvantaged status, so that I can consider the next application cycle.
A: Disadvantaged is defined as either educationally or environmentally disadvantaged. Did you work while you were in college? Did you attend a rural or inner city high school? If you went to a private school, did it have all the tools needed for you to be successful in college?
Q: Is there an online application to submit with the information or do I mail everything to the school?
A: There is an online application available from September 1 to January 15. You must mail in the required documents.
